MEES Graduate Program

STUDENT FORMS

All the MEES Program forms are listed below. Additional university-specific forms may be required for graduation. Students should check with their university representative for information about additional forms and graduation requirements for their specific university.

MEES Required Forms

All MEES Students are expected to complete and submit the forms listed below to the MEES Office upon completion of each benchmark.

• Initial Advisory Committee Report (submitted only once)

• Annual Student Progress Report (submitted annually until graduation)

• Doctoral Approved Program Form

• Report On Doctoral Comprehensive Exam

• Report On Doctoral Dissertation Proposal

MEES Optional Forms

The forms listed below should only be submitted to the MEES Office if applicable.

• Co-Advisor Agreement Form

• Doctoral Waiver of Credits Form
